To help us provide the most accurate assessment of your oral health, please bring the following information with you to your consultation appointment:
- Your referral slip and X-rays, if applicable
- A list of any medications you are taking
- Any known allergies or medical conditions
- Medical or dental insurance to be filed
What Happens During and After Your First Appointment
Occasionally, treatment can be completed the same day as the consultation. However, complex treatment plans or complicated medical histories may require further evaluation and a second appointment for treatment. Please let our office know if you have a medical condition that may be a concern prior to any procedure, such as diabetes, high blood pressure, artificial heart valves or joints, rheumatic fever, or blood disorders.
If you have had X-rays taken in the last six months, please ask your previous dentist to forward them to us. If there is not enough time to receive them before your appointment, please arrange to pick them up and bring them with you. Please also note that a parent or guardian must accompany all patients under the age of 18.
